Syracuse Community Health is a Federally Qualified Health Center founded in 1978, providing comprehensive primary and specialty healthcare to residents of Greater Syracuse, Onondaga County and Central New York. Care is offered regardless of ability to pay, social status, financial means, race or gender. The organisation operates a network of 11 health centres and is certified by the NCQA as a Patient-Centered Medical Home, a team-based model focused on achieving the best possible health outcomes.
Services span primary care, dental care, pediatrics, obstetrics and gynaecology, eye care, podiatry, quick care, pharmacy, radiology, behavioural health and school-based health. The organisation also provides telehealth, language translation and interpretation for patients, and delivers care under a whole-person philosophy. In December 2023 it opened a 56,000-square-foot facility for whole-person care, succeeding a main office that had served patients for 70 years.
