Grace Health

Grace Health is a Federally Qualified Health Center serving Calhoun County, Michigan. Founded in 1986 and grown out of the North Avenue Women's Center, it provides comprehensive, patient-centered primary and specialty care to patients ranging from newborns to seniors. The organisation operates from two locations, in Battle Creek and Albion, Michigan.

Its clinical offering spans family practice, internal medicine, paediatrics, OB/GYN, dental care, behavioural health, pharmacy, optometry and physical therapy. Grace Health also maintains health centers within local schools, extending access to students. Care is organised around a Neighborhood Model, in which primary care providers work collaboratively with other medical professionals to coordinate comprehensive treatment for each patient.

The organisation is recognised as a patient-centered medical home by the National Committee for Quality Assurance and has received quality awards from the same body. Its stated mission focuses on excellence in quality, service and access, with a commitment to serving underserved populations. Grace Health accepts Medicaid, Medicare and most insurance plans.
