Hope Hospice is a nonprofit provider of hospice and palliative care serving patients and families in the Tri-Valley and East Bay regions of Northern California. Founded in 1980 as a community-led initiative, the organisation delivers care in and around Dublin, Livermore and Pleasanton, and in surrounding communities. It forms part of the largest independent nonprofit hospice network in Northern California, alongside By the Bay Health and Mission Hospice.
Its services span Hospice Care, Palliative Care, Transitions Services, Grief Support, Caregiver Resources and Dementia Caregiver Support. Care is delivered by interdisciplinary teams that include nurses, physicians, social workers, hospice aides, chaplains, bereavement professionals and trained volunteers, addressing the physical, emotional and spiritual needs of patients and families.
As a nonprofit, Hope Hospice states that it is driven by mission rather than financial demands. Its stated approach emphasises individualised, patient-centred care, with patients and families placed first, and values consistency, responsiveness and quality of care.
