Cornerstone Hospice & Palliative Care

Cornerstone Hospice & Palliative Care is a community-based, not-for-profit provider of end-of-life hospice and palliative care for people with life-limiting illnesses. Founded in 1981, the organisation has delivered care for more than four decades, with headquarters in Tavares, Florida and a service area spanning Central Florida and North Georgia.

Its services are delivered in patients' homes, in its own inpatient facilities and throughout the community. These include comprehensive hospice care, skilled nursing available 24 hours a day, seven days a week, personal care assistance with hygiene and skin care, medical equipment and therapy services, social work support for patients and families, and bereavement and grief support for family members. Palliative care is offered alongside hospice care, and volunteer programmes provide additional assistance to patients and their caregivers.

The organisation operates as part of Chapters Health System, described as one of the nation's community-based healthcare delivery systems. Its stated commitments include a mission of compassion and dignity for patients and caregivers, and making hospice care available to all regardless of ability to pay.
