Brevard Health Alliance

Brevard Health Alliance is Brevard County's only Federally Qualified Health Center. Founded in 2005, it provides medical care to patients of all ages regardless of insurance status, operating as a patient-centered medical home with an emphasis on preventive care and disease management. The organisation serves more than 60,000 unique patients a year across seven locations in Brevard County, Florida.

Its services span family medicine, pediatrics, behavioural health, dental care, an on-site pharmacy, women's health, senior care, and specialty services including diabetes management and chronic disease care. Clinical work is organised around integrated care coordination and preventive care and chronic disease management, with behavioural health delivered as part of integrated care rather than in isolation.
