Aviva Health

Aviva Health is a non-profit Federally Qualified Health Center serving Douglas County, Oregon. It provides comprehensive primary medical, dental, and behavioral health care across 11 service locations. Founded in 1991 by a community task force as the Open Door Clinic on Pine Street, the organisation became an official FQHC in 1999 and has operated in the county ever since.

The scale of its operations has grown markedly. The clinic recorded 1,241 patient visits in its first year; in 2025 it recorded over 72,000. Central to its model is a discounted sliding fee schedule, set according to family size and income, under which no patient is turned away for inability to pay. Services are offered regardless of insurance status or ability to pay, reflecting an explicit mission to serve the medically underserved.

Aviva Health works to a patient-centred primary care philosophy. It holds the designation of Douglas County's first 5-star Patient-Centered Primary Care Home, the highest awarded by the Oregon Health Authority. Roles span primary medical care, dental care and behavioral health care within a mission-driven, non-profit setting.
