Community Healthcaring Kitchener-Waterloo (CHCKW) is a community health organization serving vulnerable populations in the Kitchener-Waterloo region of Ontario. Founded in 2021, it provides comprehensive primary healthcare, illness and disease prevention, and health promotion education to people facing barriers to health services, including newcomers and refugees, people experiencing homelessness or precarious housing, and individuals affected by poverty, food insecurity and social exclusion.

Care is delivered by a multidisciplinary team that includes Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists and Psychiatrists. Work spans clinical practice and the wider social determinants of health, with an emphasis on culturally sensitive and language-accessible care. Services are evidence-informed and designed to be innovative in addressing health inequities.

CHCKW serves approximately 23,156 clients and records more than 44,000 provider visits annually. Its work is concentrated in Kitchener-Waterloo and the wider Waterloo Region. Roles at the organisation sit within community health, primary healthcare and refugee and newcomer settlement health, in a setting built around collaborative, team-based delivery.
