MedZed

MedZed combines community health navigators and proprietary technology to reconnect high-risk Medicaid and Medicare members with primary care and address social determinants of health across 11 US states.

MedZed is an American healthcare services company founded in 2014 that works to close gaps in care for high-risk Medicaid and Medicare members. Its model combines locally-based Community Health Navigators - care workers who meet members where they are, including in their own homes - with proprietary care management technology. The navigators pursue persistent, empathetic engagement with people who have disengaged from the health system, rebuilding trust and reconnecting them to primary care while addressing social barriers such as housing instability, food insecurity and gaps in insurance coverage.

The company's principal services are Enhanced Care Management, delivered by navigators in members' homes, and Community Supports, a set of non-clinical services aimed at social determinants of health. Enhanced Care Management is reported to produce a 40% reduction in emergency-department visits and inpatient admissions per thousand members. Underpinning these services is in-house work on care management technology, healthcare data and outcome analytics, and remote and home-based member engagement platforms.

MedZed serves more than 50,000 high-risk individuals across 11 states in partnership with 22 health plans. Its clients include Medicaid managed care organisations, Medicare Advantage plans and risk-bearing provider organisations. Operations are concentrated in the United States.

The company describes itself as mission-driven, focused on the most vulnerable members of the healthcare system, and oriented toward community-based, trust-based care delivery.
