HealthArc
HealthArc provides an AI-powered cloud platform for chronic care management, offering nine CMS-reimbursable care programmes, EHR and device integration to US providers and payers.
HealthArc operates a digital health platform built as an AI-powered operating system for chronic care management. The company supplies integrated infrastructure to healthcare providers and payers - among them physician practices, health systems, virtual care companies and health insurers - intended to support the scaling of care delivery. Its cloud-based software covers remote patient monitoring, EHR integration, healthcare interoperability and medical device connectivity, and it combines that technology with managed clinical services.
The platform carries nine CMS-reimbursable programmes on a single system: Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM), Transitional Care Management (TCM), Principal Care Management (PCM), Behavioral Health Integration (BHI), Advanced Primary Care Management (APCM), Medication Therapy Management (MTM) and Principal Illness Navigation (PIN). The stated design aim is to replace disconnected point tools with one operating environment, improving operating efficiency and maximising programme reimbursements.
HealthArc reports more than 2,000 providers connected and 100,000 patients served across 45 or more US states, with over 40 connected medical devices integrated. Organisations can adopt the system in three ways - fully managed, co-managed or software-only - allowing customers to choose how much of the clinical service layer they run themselves.
