Ennoble Care

Ennoble Care is a community-based provider of primary, palliative and hospice care, delivering personalised in-home medical services to people with chronic conditions and limited mobility. Its physicians travel to patients' homes, senior living communities and long-term care facilities for housecalls; the company also offers palliative care focused on symptom relief for serious illness and hospice care for terminal patients.

Beyond in-person visits, Ennoble Care runs programmes in remote patient monitoring, telemedicine visits, chronic care management, medication management and behavioral health management. The aim of the model is to enable seniors to age comfortably in place, supported by continuous and coordinated care rather than episodic office visits.

The organisation is physician-led and works through multidisciplinary teams that include nurse practitioners, physician assistants, registered nurses and certified home health aides.
